Q666. How does EPA define “identification of the facility or supplier” in 98.4( i )(1) with regard to a facility’s certificate of representation?

 

A666. Per 98.4, facilities and suppliers are required to complete a certificate of representation that includes an identification of the facility or supplier, along with other information. The identification to be provided on the certificate of representation is the facility or supplier’s physical street address (not a mailing address). The physical street address is necessary for identification because some facilities and suppliers have similar names. Providing a facility or supplier’s address on the certification of representation also fulfills the requirement in 98.3(c)(1) to report the facility or supplier’s physical street address.
